SHS Transition Services

   Our Mission:

To ensure that students ages 16-21 are given the training, experiences, and supports needed to function independently as they transition from SHS to post-secondary education and training, community access, independent living, and employment.

Transition Services include:

  • Individual Planning, focused on post-secondary IEP goals
  • Instruction in academic, vocational, independent living, and personal social skills
  • Career Development Services including goal setting, career exploration, job search skills, and self advocacy
  • Emphasis on Paid Employment such as connections with local employers, operation of school based businesses, on the job training and assessments
  • Information and Referral Services: Vocational Rehabilitation, DD Services, SSI, etc.
